Actionable steps to reach isolated seniors and improve aging in place generated so far include:

1. Promoting and creating opportunities for regular, local interactions

2. Ramping up transportation and home visits

3. Centering on relationships for health and wellbeing

4. Ensuring the availability of options for diverse expressions

5. Harnessing technology for connections among service users

6. Harnessing technology for connections among service providers

7. Connecting the formal and informal care systems
